2025 got off with a strong start offering new information for those who have diabetes. Since it is one of the most common chronic diseases in the US, and around the world, interest and research in the field of diabetes remains high.
Let’s examine which important diabetes information is currently being discussed.
Technology Explosion in Diabetes
Diabetes technology evolves quickly. There is more data stating that “education of obesity treatment, type 2 diabetes prevention and use of diabetes technology is extremely important in helping to take control of the diabetes epidemic.”
The OTC CGM (continuous glucose monitor) will be one of the biggest diabetes trends of the year. It “offers a better quality of care for people with diabetes.” Instead of “by prescription only” for those with type 1 or those with type 2, only on insulin, CGM has become an effective tool for those with type 2 diabetes on pills and even for people without diabetes who just want to track their blood sugars.
Prior to this trend, you generally spoke to your endocrinologist about wearing a CGM. Now your primary care doctor or internist should be familiar with these products.
There is a new CGM, the Eversense 365, from Ascensia Diabetes Care which is an implantable CGM worn for 365 days.
What is Eversense 365 CGM?
- A tiny sensor is implanted under the skin of the upper arm, by a healthcare professional.
- A removable transmitter sends glucose readings to a mobile App.
- It shows glucose data updated every 5 minutes.
- It sends a vibration alert when glucose levels are high or low. It is “considered very accurate.”
- It requires a prescription but both most commercial health care plans as well as Medicare covers the Eversense365 CGM.
- There are rare skin reactions to the adhesive, less skin sensitivity to this product, less problems of the device falling off.
What is the Omnipod 5 automated insulin delivery system?
The first insulin pumps that were for commercial use were created in 1978, but they were much bigger, more difficult to operate and not automated. Omnipod, the wireless pump, had its first clearance in mid-2003 and the first commercial version launched in 2005.
In January 2022, the Omnipod 5 became the first automated and tubeless pump with “specific software to regulate blood sugars.” It is integrated with the DexCom G6, G-7 CGM and the Freestyle Libre 2 CGM. It is approved for type 1 diabetes patients over age 2 and type 2 diabetes patients over age 18.
The automated insulin delivery system works for 3 days without multiple finger sticks or daily insulin injections. The Omnipod 5 is shown to “lower A1C levels and keep you more time in range.” It is easy to self-apply and can be worn under your clothing.
Smart Adjust technology is built into the Omnipod-5. The brain of the system keeps you in range day and night. It adjusts insulin delivery automatically, up or down, and pauses every 5 minutes. The Omnipod 5 predicts future glucose values based on previous trends.
What is the Twiist AID – Automated Insulin Delivery System?
Twiist AID is noted as “not just a new insulin pump delivery system but a new era of diabetes management.” It received FDA clearance on March 18,2024 and will be distributed through multiple pharmacies. It will provide personalized diabetes management from an I-phone, an Apple watch or using an on-pump bolus button. The Twiist allows for pre-meal and workout pre-sets. What is the new insulin in 2025?
75 million people with diabetes inject insulin. The newest insulin will be ready to be tested on humans in 2025 trials and will be led by Endo Axiom Pty Ltd. The oral insulin has already been tested on baboons and “it lowered their blood sugar, with no episodes of hypoglycemia.” It is a smart insulin which will be delivered in a capsule.
Insulin within the “nano-carriers” allow it to be delivered to areas of the body that need it the most. There is also a coating within the capsule to protect the insulin in the digestive tract from stomach acid and digestive enzymes.
The smart insulin will be safe until it gets to the liver. The coating will only break down when blood sugar is high. If blood sugars remain low, no insulin will be released from the smart capsule. It will not need to be refrigerated.
Immunotherapies for Diabetes
Immunotherapy is already used for certain autoimmune diseases and some forms of cancer. Immunotherapy for diabetes will, “prevent your immune system from attacking insulin producing cells.” Hopefully it will target beta cells in the pancreas, where insulin is produced. This may eliminate type 1 diabetes.
Some trials have already been done in the UK, and it did stop the onset of diabetes for a few months. The goal is to give immunotherapy before diabetes starts, as prevention, or to people newly diagnosed to slow down or halt the attack on the beta cells of the pancreas.
Type 2 diabetes remission
Another promising area of research in 2025 involves “remission of type 2 diabetes without the need for diabetes medication.” Remission means patients will revert to normal blood sugars, 60-99mg/dl fasting and up to 140 mg/dl, 2 hours after meals.
One of the trials called The Direct Trial tests a low-calorie diet along with a weight support coach (from a healthcare professional). At the end of the first year,” half the people were in remission.” Remission may not be possible for everyone, but more studies and trials are being done. It will take time.
Engineering Beta cells in the Lab
There is active research on creating beta cells (which produce insulin) in the pancreas in the lab setting. The goal is for the beta cells to be replaced when they stop working. There are also studies and trials being done to reboot “the actual beta cells in your body” when they stop working. The present work involves Sorcin, a molecule that can protect beta cells, from becoming exhausted and keep them healthy to continue to produce insulin.
What is the most important habit to establish and maintain in 2025 to increase your longevity?
Aging is the natural deterioration of physical and biological functioning. Research shows “a regular exercise program is the most influential daily habit for improving longevity.” Longevity simply refers to “long life.” Exercise can add about 2-4 years to your life expectancy and is a strong predictor of longevity.
Obviously, it also depends on genetics, which accounts for 25%, other lifestyle habits and your environment. Not only do you want a longer life, but you want to maintain a high quality of life.
The average lifespan in 1900 was 47 years. People born in 2025 can expect to live to an average of 80 years. Physical activity is any movement that uses energy, and exercise is a structured type of physical activity that aims to improve fitness.
Why is exercise the link?
Regular physical exercise reduces inflammation which is associated with chronic disease and diabetes. It improves blood circulation, lung and heart function, improves balance, helps your emotional mood, improves sleep patterns, speeds up metabolism, helps regulate blood sugars, builds muscle mass, helps you maintain a healthy weight, boosts brain health, lowers stress levels, raises endorphins (feel good hormones) and reduces the risk of developing diabetes.
The current suggested Physical Activity Guidelines were written in 2008, and they remain the same in the update of 2020. These guidelines are used by the CDC, FDA and the WHO organization.
- 150 minutes of moderate aerobic activity. 30 minutes or 5 days a week or more.
- 2-3 days of muscle strengthening activity. 30 minutes each session.
- If you can’t be as physically active as this, “be as physically active as your abilities and conditions allow. Do not remain sedentary.”
- Make exercise a part of your daily life.
